Forms and Information

If you have already registered, the rest of these forms will need to be filled out prior to attending the trip.
**Note: Bus Form must be notarized, so don’t wait till the last minute!**
**NOTE: The forms below are currently the 2025 forms for reference. When we get the 2026 forms, this note will go away.**

“What to Bring” and “Dress Code” lists can be found in the Student packet above. Also, since we will traveling on FBC buses, everyone will need to fill out (and have notarized) the Bus Form above.

“Sample Schedule” will give you some idea of what to expect.
(Don’t forget to bring money for 2 dinners out with your group!)

How to Pay

Remember, total amount per person is determined by date of registration.

EARLY - Before May 1 - $185
STANDARD - May 1 - June 1 - $200
LATE - June 2 - Day of MPACT - $215
